Hello Camp Families!
Thank you for your Camp Otonwe registration. We look forward to providing a positive and memorable camp experience for your camper.
Please download, complete and bring the health form, parent consent form, swimming ability card and Leisure Card with you on the first day of camp. DO NOT MAIL TO THE OFFICE.
If you have any questions concerning your child’s stay at Camp Otonwe or if you need to have these forms sent to you, please call the Camp Fire office at 614-481-8227 or email lisa@centralohiocampfire.org. Your call may be forwarded to the Camp Director for response.
Otonwe confirmation 2012 ~ Please read carefully (all the pertinent information regarding rules and polices, packing list, as well as tips and guidelines)
Otonwe Health form 2012 ~ Read, complete and bring to camp. (A three page form that must be filled out and signed by the parent/guardian. Campers will not be admitted to camp without this completed form.)
Consent form ~ Read, complete and bring to camp. (A two page form that gives consent from a parent /guardian for your camper to participate in camp as well as important contact details. This also lists other persons that will have permission to pick up your child at camp. Campers will not be admitted without this completed form.)
swimming ability 2012 ~ Read, complete and bring to camp. (Please provide a brief summary of your child’s swimming abilities.)
LEISURE CARD APPLICATION 2012 ~ Read, complete and bring to camp. (for access to the swimming pool)
ACA transportation standards 2012 ~ Read carefully, for transportation to and from activities that are not on camp property.
